The immunizations required for all students are as follows:

-3 doses of DTaP, DTP or DT vaccine, one given on or after the 4th birthday
-3 doses of Polio vaccine
-2 doses of MMR vaccine, given on or after 12 months of age and separated by at least one month
-3 doses of Hepatitis B vaccine
-2 doses of Varicella (chickenpox) given on or after 12 months of age OR written documentation (including year) of varicella disease
from a parent or guardian, and are availible in the nurse's office

-7th grade: Need one dose of Tdap. This can be received any time after 10 or 11 years
depending on which brand of vaccine is received.

 

If your child does need one of more immunizations, these may be obtained at your physicians office. There is an immunization
clinic held the first Thursday of every month at the Gothenburg Memorial hospital. Call 537-3651
